CareRxConnect™ helps you better understand and manage your medicationsPCI offers CareRxConnect™, a service that helps individuals better understand and manage their medications. Our clinical pharmacists have an important role along with physicians to ensure patients receive maximum benefit from medications. When a pharmacist is added to the care team, patient have a greater understanding of why, when, and how to take medications. This helps to improve health and reduce return visits to the hospital. Our clinical pharmacists help to educate and answer questions such as:

  • Why am I taking these medications?
  • Are my medications working the way they should?
  • Are these the best medications given my clinical conditions?
  • Can I do anything about side effects?
  • Am I taking my medications in the right order, at the right time of day, can I eat food with them?
  • Is it possible that there will be potential adverse interactions among the medications I am taking?
  • How safe and effective are the vitamins or over-the-counter drugs that I am taking?

CareRxConnect™CareRxConnect™ starts with our uniquely qualified clinical pharmacists and pharmacy technicians, who contact patients in their home or residential setting. Working as a member of the care team and in consultation with patients' physicians, our pharmacists create individualized medication plans that are coordinated among all physicians and which matches clinical conditions. The CareRxConnect™ service includes:

  • A full review of all medications, including vitamins and over-the-counter products, that are currently being prescribed or taken
  • The creation of a personalized medication record with an action plan designed to avoid adverse drug interactions and to reduce unnecessary medications and cost
  • Documentation and follow-up to assist in patients continuing to receive the most effective and cost-efficient medications available
  • Patient access to the PCI pharmacist team for questions
  • Frequent PCI communication and referral to other members of your health care team when needed

The PCI program is endorsed by health plans and physicians and it may be a covered insurance benefit.
